What is a Blank Doctor Note?

When employees are in need of doctors notes to be excused from work and still get paid, they sometimes resort to just securing a fake doctor’s excuse online instead of heading to the hospital and actually requesting for one from a real physician as this takes more effort and time. After all, purchasing online and printing it is a lot more convenient than asking and requesting for a real one, especially when you’re reason for taking the day off is not as critical and you are just feeling tired.

For some people who do not want to spend more for their excuse letters, they look or search for a blank doctor note, which are offered for free by some websites. It is a doctor’s note or excuse that is offered without the actual body being written by the doctor. There are just blanks that you fill in with your information and reason for excuse. Just like any other doctor’s note, your blank doctor note should contain the following information: the name of the physician, the name of the clinic, the address of the clinic, details about the patient such as complete name and reason for the absence. It should also include the full name and signature of the attending physician.

When choosing a blank doctor note to use, it is important to use the following six criteria. First, you have to check for the logo of the clinic or hospital. It should carry a valid-looking logo or seal to make it look authentic. Second, the note should have sufficient area to write the address and phone number you would want to use and customize it accordingly. Third, you have to make sure that it does not have any typographical errors on it and that it is clean and neatly formatted and printed. Fourth, it has to have versatility of format. Fifth, the blank doctor note should have been pre-stamped with the physician’s stamp, and make sure that the stamp looks valid. Sixth and last, you have to check if it has a line for the signature if you will be signing it later on, or it must already have a fake doctor’s signature if you wish to just edit the body of the note.

It is also important to keep in mind that if you get caught using a blank doctor note with a forged signature, or a non existent doctor’s name, you may face dire consequences such as immediately getting fired from work or you may even be involved in a legal case as forgery is a crime that can cause you large amounts of fines and/or time in jail. If you are thinking of using a fake doctor note and signing it in behalf of your doctor, or if you plan on using fake doctors’ names with fake signatures, think again. You may get away with it and take the easy way out, but there will always be an equal chance that you will get caught.
